It asserts that councils, health bodies and providers need to work more collaboratively to personalise and integrate service delivery across health and adult social care; and make vital public funding go further. It also recognises the contribution that individuals, families, carers and communities make in providing care and support – both to those who are publicly funded and those who either pay for themselves or rely on family carers.</p>

It outlined a vision for Adult Social Care which focused on more personalised care with better choice and control for the citizen.</p>
<p>Since then, local councils have been working on transforming their Social Care systems. An Adult Social Care Reform Grant of Â£520 million was created to help fund the process of change for Local Authorities between 2008 and 2011.</p>
